All forms are available by registering in our client portal. All potential and active clients will be sent a link for registration. The portal allows you to complete and sign all forms electronically. The only document you will see listed here is the HIPAA Privacy Policy, which should always be available for client review. Once intake paperwork has been completed, our office will receive a notification and you will have a copy of the signed document held within the portal for your review as well. The client portal is of a great benefit to our families as it allows you to view appointments and forms, insurance and billing items as well as allowing you to make a payment, should you have a balance due.
